The consumer, parent, or legal guardian will be asked to sign a
consent so that specific questions may be asked regarding
consumer's diagnosis, psychological/psychiatric histories,
hospitalization, social history, education, vocation, and medical
history. The Case Manager, along with the Interdisciplinary Team, will
meet with the consumer and the Provider of Choice to discuss the
consumer's needs, strengths, and goals and to help the consumer with
deficits and assist them in becoming as independent as possible. If
Uprising Homes is chosen as the Provider, the guardian or consumer
must complete a screening assessment with identifiers to determine
whether or not the organization can provide the needed services. If
Uprising Homes determines that services can be provided, then the
consumer will assigned a worker. A consumer satisfaction survey will
be completed quarterly to annually to assess the consumer's
satisfaction of services.
